Transitioning to the Community
The background
"M" came to Jacaranda following a long-term stay in a secure mental health setting. He was highly anxious about managing his own time and interacting with the community.
Our approach
Our 24/7 team provided structured daily routines, starting with 1-to-1 support for local walks and grocery shopping. Through our weekly psychology sessions, M learned to manage his anxiety triggers safely.
The outcome
Within 12 months, M is now independently managing his medication, cooking his own meals twice a week, and has recently started a part-time volunteer role at a local charity shop in Reading.
